Fill in a trip consent form

Give your child’s health details, an emergency contact and your permission.

1. Find the trip

Tap Trips at the top of the page. Or go to trips.bmapay.org.
Each trip shows the date, the place, the price and if places are left. Full means there are no places left: you can join the waiting list.
Tap Pay now or Details.
2. Open the form

Some trips need a consent and medical form before you can pay. You fill in one form for each child.
Tap Fill in the form next to your child’s name.
3. Health details

Tell us about medical conditions, medication, allergies and food needs.
If there is nothing to tell us, write None.
Only staff running the trip can see these details.
4. Emergency contact and permission

Give the name and phone number of someone we can call in an emergency.
Tick Save these details if you want them filled in for you on the next trip.
Tick the box to give permission. Then tap Save and continue.

To change the form later, tap Change next to your child on the trip page.

The pictures come from a test account. Your names, dates and prices will be different.
